Health Assessments

At our surgery we offer comprehensive and individualised GP Management Plans which are completed by a nurse and reviewed by a GP for patients diagnosed with a chronic health condition. Some care plan patients are eligible for referrals to allied health providers at a subsidised cost. We also offer health assessments for over 75's, asthma plans, healthy heart checks, anaphylaxis action plans and diabetes reviews. Please ask your doctor if you are eligible for a care plan.
